Continuity of care is an assumption that many of us make far too often when immersed in health care issues.  Unfortunately, the care continuum is often broken as patients move across both medical and non-medical settings.  Too often, patients land in “no care zones,” where there is little or no coordination between those who were providing care and those who are about to assume it … family caregivers.
